An ipsilateral pneumothorax is a known complication of CIED implantations. A contralateral pneumothorax from a left-sided device implant is uncommon and has been associated with RA lead perforation. How can bilateral pneumothoracies after implantation occur without lead perforation? We describe a case of Buffalo Chest Syndrome that was discovered coincidentally while managing a same-side pneumothorax after an ICD placement.
