This article addresses the advances and challenges of the Psychosocial Care Network over more than 35 years of psychiatric reform in Brazil. Its main objective is to analyze the services provided by the network, identifying the key obstacles to the effective implementation of its guidelines. The research uses an integrated literature review, examining 28 articles selected from an initial total of 1,650 publications, with the aim of offering an updated panorama of mental health in the country. The analysis reveals significant issues, such as the lack of coordination between network services, difficulties in promoting users' autonomy, and deficiencies in the training of professionals aligned with the principles of psychosocial rehabilitation. Despite some progress, service coverage remains insufficient in vulnerable areas, compromising the principle of equity. The study highlights the need for strategic planning to improve the Psychosocial Care Network functionality, strengthen intersectoral integration, and invest in the training of mental health professionals. The research provides a comprehensive overview of the system's main bottlenecks and suggests actions to consolidate a more inclusive and humanized community-based care model.
