Abstract Background The ENEIDA and UR-CARE registries are major European platforms for clinical characterisation and research in inflammatory bowel disease (IBD). Their recent synchronisation represents a major step towards cross-national data harmonisation. The V-SYNC study evaluated whether synchronized ENEIDA and UR-CARE data produce identical outputs when executing clinically relevant queries. The aim of this analysis was to functionally validate the synchronisation by assessing whether both registries generate identical outputs when running predefined quality-of-care queries. Methods We applied ten clinically relevant queries—covering treatment patterns, safety screening, comorbidities, pregnancy, surgical outcomes and smoking status—to both databases. Queries reflected key ECCO quality standards and included: (1) number of patients on advanced therapies; (2) proportion screened for hepatitis B, hepatitis C, HIV and tuberculosis before first advanced therapy; (3) prevalence of osteoporosis and osteopenia; (4) annual number of pregnancies; (5) postoperative complications within 30 days; (6) initiation of postoperative prophylaxis after ileocaecal resection; (7) initiation of infliximab with immunomodulators; (8) proportion switching to a second biologic for primary non-response; and (9) current smokers. Query outputs from ENEIDA and UR-CARE were compared for numerical equivalence. Results Results from all functional queries were fully concordant between ENEIDA and UR-CARE (table 1). Conclusion Functional testing demonstrated complete concordance across all ten quality-of-care queries, confirming the technical robustness and reliability of the ENEIDA–UR-CARE synchronisation. These results validate the interoperability of the two registries and support their combined use for multinational research, quality benchmarking and future data-harmonisation initiatives.
