Abstract Background Home parenteral nutrition (HPN) is a life-sustaining therapy for selected inflammatory bowel disease (IBD) patients with intestinal failure. Mortality data in this setting remain limited. This study aimed to describe the mortality patterns and causes among IBD patients receiving long-term HPN. Methods All adults with Crohn’s disease or ulcerative colitis who were treated with HPN between 2005 and 2025 at a tertiary centre were retrospectively reviewed. Dates of HPN initiation, discontinuation, and death were recorded. Deaths were classified as IBD- or non-IBD-related. For IBD-related deaths, HPN duration until death was calculated. Results Seventy-six patients were included (48 males, 28 females; Crohn’s 68, UC 8). Median age at HPN start was 36 years (18–78). Overall mortality was 6/76 (7.9%). Of these, 4 IBD-related deaths occurred during active HPN, 1 non-IBD-related death during HPN, and 1 unknown cause occurred in another facility. For IBD-related deaths (n = 4), mean HPN duration before death was 21.4 ± 30.0 months (median 9.4; range 1.5–65.3). Conclusion In this tertiary IBD cohort, four patients died of IBD-related complications, mainly sepsis or malignancy. All deaths occurred in Crohn’s disease. Vigilant multidisciplinary follow-up, infection prevention, and malignancy surveillance are essential to reduce mortality.
