Background:In December 2025, India reported two confirmed cases of Nipah virus (NiV) infection in the state of West Bengal, prompting international concern and heightened surveillance measures across Asia. This outbreak occurred amid increased global awareness of pandemic threats following the COVID-19 pandemic, raising questions about NiV’s pandemic potential. Methods:A comprehensive epidemiological analysis of the West Bengal outbreak was conducted, examining case characteristics, contact tracing outcomes, and public health response measures. A comparative risk assessment evaluated NiV against SARS-CoV-2 (COVID-19) across key pandemic determinants, including case fatality rate (CFR), basic reproduction number (R₀), transmission mechanisms, and healthcare system responses. Results:The outbreak involved two healthcare workers, with epidemiological evidence suggesting nosocomial transmission. Among 196 traced contacts, all tested negative and remained asymptomatic, indicating successful containment. NiV exhibits a high CFR (40–75%) but a low R₀ (<1.0), in contrast to SARS-CoV-2’s lower CFR (0.5–2%) and higher R₀ (2–6). Paradoxically, NiV’s high virulence limits its pandemic potential by reducing opportunities for transmission through rapid case fatality. The outbreak was confirmed as a natural zoonotic spillover from fruit bats, not a laboratory-associated event. India’s transparent reporting and rapid public health response stood in contrast to the early management of the COVID-19 outbreak. Conclusions:The West Bengal NiV outbreak poses minimal pandemic risk due to key virological constraints: limited human-to-human transmission (R₀ <1.0), high case fatality preventing sustained transmission chains, absence of a respiratory-dominant phenotype, and effective containment measures. Unlike SARS-CoV-2, NiV lacks pandemic-enabling characteristics. Nonetheless, NiV remains a high-consequence pathogen requiring continued vigilance, improved diagnostics, therapeutic development, and strengthened One Health surveillance in endemic regions.
