Background: Diffuse midline glioma (DMG), H3 K27-altered, is an aggressive paediatric high-grade glioma with poor prognosis. Although typically presenting with rapid clinical deterioration, indolent precursor lesions are rarely documented. We describe an unusual case of a brainstem DMG arising from a long-standing, radiologically stable lesion followed for seven years prior to accelerated progression.
