Abstract Lesions of the caruncle are rare, clinically diverse, and often challenging to diagnose. We report a rare case of a unilateral caruncular dermoid cyst in an 18-year-old female who presented with a slowly progressive, painless, whitish, avascular mass measuring approximately 1 × 1 mm at the left medial canthus. Ophthalmic examination revealed no ocular or systemic abnormalities suggestive of syndromic associations, such as Goldenhar syndrome. Surgical excision under topical anaesthesia was uneventful, and histopathological analysis confirmed the diagnosis of a dermoid cyst. Given the broad differential diagnosis of caruncular lesions, definitive histopathological evaluation remains essential. Close interdisciplinary collaboration between ophthalmologists and pathologists is crucial for accurate diagnosis and optimal management of such uncommon lesions.
