A Case of Laparoscopy-assisted Partial Colectomy for Descending Colon Cancer in a Patient on Peritoneal Dialysis

Abstract

症例は47歳の男性．慢性腎不全で4カ月前から腹膜透析（peritoneal dialysis;以下，PDと略記）が開始された．生体腎移植前のスクリーニング目的に施行された下部消化管内視鏡検査で，SD-junction付近の結腸に深達度M～SM程度の早期癌を指摘された．非PD患者であれば内視鏡的切除も考慮される病変であったが，PD患者に対する内視鏡的治療は腹膜炎のリスクが高いと判断され，手術目的に当科へ紹介された．術前診断は，S状結腸癌または下行結腸癌（cTis～cT1，N0，M0，cStage 0 or I）で，腹腔鏡補助下結腸部分切除術を行った．術翌日にブラッドアクセスカテーテルからの血液透析を開始し，術後9日目にPDを再開した．PDは問題なく施行可能であり，術後11日目に退院した．日本透析医学会より腹膜透析ガイドラインが刊行されているが，PD患者に対する腹部手術の周術期管理方法の記載はない．PD療法中の大腸癌手術に関して，周術期管理法を中心に文献的考察を加えて報告する．
