Objectives: To evaluate surgical management outcomes in advanced-stage oral submucous fibrosis (OSMF). Methods: Three advanced OSMF cases (two Stage IV and one Stage III) were managed surgically using fibrotomy combined with nasolabial flap reconstruction, buccal fat pad grafting, collagen sheets, or skin grafts. Preoperative and postoperative mouth opening measurements were recorded. Results: Two patients demonstrated significant improvement in interincisal distance (30 mm and 35 mm). One patient showed relapse due to poor postoperative compliance. Healing was uneventful in all cases without major complications. Conclusions: Surgical intervention remains the treatment of choice in advanced OSMF. Regional and autogenous flap techniques provide effective functional improvement. Long-term follow-up is essential due to the premalignant nature of the disease.
