Abstract:This paper reports a case of a 77-year-old elderly patient with type 2 diabetes mellitus complicated by multimorbidity. By applying the Comprehensive Geriatric Assessment (CGA) technology, the multidisciplinary collaborative team accurately identified the high-risk factors of the patient, and organized and implemented an individualized comprehensive intervention plan centered on blood glucose control and prevention of disease deterioration. During hospitalization, through targeted treatment and nursing care, the patient's blood glucose tended to be stable, the redness and swelling of the left foot gradually subsided, no open foot disease or related complications occurred, and the patient was discharged with improvement. This case provides a reference for the clinical nursing of elderly patients with diabetic foot complicated by multimorbidity, and explores the establishment of a whole-process nursing model of "risk prediction - stratified intervention - family empowerment - continuous guarantee".
