Thoracoabdominal aortic aneurysms (TAAAs) remain a major therapeutic challenge due to their anatomical extent and the need to preserve visceral perfusion. The Multilayer Flow Modulator (MFM®) is an endovascular alternative designed to modulate blood flow, promoting thrombosis within the aneurysm sac while maintaining patency of branch vessels. We report a case of treatment failure following MFM® implantation, leading to aneurysm progression, open surgical conversion, and death due to multiorgan failure.
