Abstract To present a rare case of persistent sciatic artery (PSA) complicated by arteriosclerotic occlusive disease and discuss the diagnostic and therapeutic challenges. Methods: Retrospective analysis of clinical findings, imaging results, and surgical interventions in a 61-year-old female presenting with left leg numbness and intermittent claudication. Imaging revealed PSA with concurrent stenosis of the external iliac and femoral arteries. Treatment involved endovascular stenting followed by open surgical intervention, resulting in significant symptom improvement. This case highlights the importance of a multidisciplinary approach in managing rare vascular anomalies, integrating endovascular and open surgical techniques to achieve optimal outcomes.
