This study analyses the impact of CSR in the health sector of India’s North Eastern Region (NER) between 2014–15 and 2021–22, using a mixed-method approach. Quantitative analysis of Ministry of Corporate Affairs data and qualitative case studies reveal stark disparities between NER and the rest of India. While NER received only ₹752.66 crores compared to ₹24,534.26 crores elsewhere, its per capita CSR health spending was higher, indicating efficient fund use. Intra-regional variations are significant, with Assam alone receiving 82.5% of NER’s health CSR funds, raising concerns about equity in distribution. CSR allocations are positively linked to industrial activity, but constraints such as limited corporate presence, geographical barriers, weak local partnerships, and information gaps restrict progress. Nonetheless, opportunities exist in preventive healthcare, technology adoption, and public-private partnerships. A longitudinal review of SDG 3 shows upward trends, particularly in Mizoram. The paper recommends targeted allocation, regional CSR hubs, standardized impact frameworks, and strategic interventions for equitable health outcomes.
