ABSTRACT Transcatheter mitral valve interventions have emerged as valuable alternatives to surgical repair in high‐risk patients, including transcatheter closure of mitral valve perforations. However, there is little experience with percutaneous mitral valve interventions in pediatric patients or patients with congenital heart disease. We report a case of transcatheter closure of a residual mid‐leaflet mitral valve cleft in a 19‐year‐old patient with moderate mitral regurgitation through two defects in the anterior leaflet of a repaired cleft mitral valve using a retrograde approach and a novel device, a Siege Vascular Plug (Merit Medical Systems Inc, UT, USA), with favorable results.
