This preprint presents the SIC-HR (Systemic Incoherence of Cardiac–Hydrodynamic Rhythms) framework, a theoretical model for migraine that reframes the disorder as a dynamic regulatory process rather than a discrete pathological event. The model integrates cardiovascular rhythms, cerebrospinal fluid dynamics, and predictive brain mechanisms to explain the temporal accumulation of unresolved activation states preceding migraine attacks. SIC-HR does not aim to replace existing vascular, neurogenic, or CGRP-based models, but to provide a unifying perspective that accounts for timing-dependent symptom emergence, variability in pharmacological response, and the presence of systemic manifestations beyond pain.
