Key Points

  • This report focuses on the diagnosis and treatment strategies for idiopathic granulomatous mastitis, a condition with overlapping features of breast carcinoma.
  • Analysis of clinical features and diagnostic challenges associated with idiopathic granulomatous mastitis.
  • Utilization of fine needle aspiration cytology and core needle biopsy for diagnosis.
  • Assessment of treatment modalities, including steroids, immunosuppressive medications, and surgical options.
  • Fine needle aspiration cytology can yield misleading results in some IGM cases.
  • Core needle biopsy or excision biopsy confirmed the diagnosis of IGM.
  • Surgery is recommended for patients not responding to medical treatments, also addressing any coexisting invasive carcinoma.

Abstract

Abstract Idiopathic granulomatous mastitis (IGM) is a chronic granulomatous disease of the breast with unknown etiology. Clinical features of IGM overlap with features of breast carcinoma. Fine needle aspiration cytology may be misleading in a few cases. However, a confirmatory diagnosis is made from the core needle biopsy or excision biopsy. Steroids and immunosuppressive medications are the primary modality of treatment. Surgery is the better option for a patient who fails to respond to steroid or immunosuppressive treatment. Surgery will remove the disease pathology, and sometimes it may also remove invasive carcinoma, which may occur concurrently with IGM.
