Abstract

This paper argues, on the basis of convergent primary fMRI, PET neuroimaging, pharmacological blockade, and meta-analytic evidence, that a structured 90-minute mechanistic psychoeducational protocol constitutes a pharmacologically equivalent biological intervention — not preparatory to such intervention, but identical with it. Three parallel mechanistic pillars are established: (1) verbal mechanistic explanation as a precision affect-labeling task activating RVLPFC–MPFC top-down inhibition of the amygdala (Lieberman et al., 2007; Wood et al., 2015); (2) biological reframing eliminating social-evaluative threat — the dominant driver of CRF-mediated FAAH upregulation and anandamide depletion — permitting ECS restoration (Dickerson Gray et al., 2015); and (3) installation of hope via pathways and agency thinking directly shifting endogenous neurochemistry from KOR/dynorphin dysphoria to MOR/DA/CB1 safety signaling (Scott Benedetti et al., 2011). A four-element clinical protocol — comprising neuroaffective compass mapping, attachment timeline construction, drama triangle analysis, and attachment-style profiling — is mapped onto these three pillars using Dual-Channel Mechanistic Transmission (DCMT): simultaneous verbal and visual delivery targeting prefrontal cortical and subcortical limbic/PAG processing in parallel.
