Abstract

District hospitals in Uganda play a critical role in healthcare delivery but face challenges in cost-effectiveness. A systematic literature review was conducted to identify relevant studies on district hospitals' operational costs, patient outcomes, and resource allocation. The analysis employed a Bayesian hierarchical model to estimate the cost-effectiveness ratios (CERs) across different districts. Bayesian hierarchical modelling revealed significant variations in cost-effectiveness among districts, with some showing substantial inefficiencies in resource management compared to others. The Bayesian hierarchical model provided nuanced insights into district hospital systems' performance and highlighted areas for improvement through targeted interventions. Policy recommendations include the use of evidence-based decision-making tools such as CERs to guide investment decisions, focusing on enhancing cost-effectiveness in underperforming districts. Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
