Abstract

要旨 【目的】 熱中症において播種性血管内凝固(disseminated intravascular coagulation: DIC)の合併は死亡率に関連する。本邦では2024年に,深部体温40℃以上かつGlasgow coma scale ≤8点を最重症群のIV度と定義した。本研究は熱中症IV度における重症度とDICの合併率を明らかにし,凝固障害の推移および生存転帰を明らかにすることを目的とした。 【方法】 本研究は2015年1月1日から2024年12月31日までに,関西医科大学附属病院に搬送され,集中治療室に入室した熱中症IV度の35例を対象にした後方視的観察研究である。 【結果】 35例の中でDICと診断されたのは19例(54.2%)であった。DIC群で収縮期血圧の低下,乳酸値と血清クレアチニンの上昇,sequential organ failure assessment(SOFA)スコアの上昇を認めた。入院時の血液検査で2群間に凝固障害に差はなく,DIC群で24時間以内にPT–INRの延長,Dダイマーの上昇,血小板減少,フィブリノゲンの減少がみられた。2群間で死亡率に差は認めないが,入院期間が延長した。 【結語】 熱中症IV度の54.2%がDICを合併し,IV度はDIC合併の高リスク群と考えられ,ショックはよりDICを来す可能性が高いことが示された。
