ABSTRACT Bilateral simultaneous spontaneous pneumothorax is a rare occurrence that generally requires prompt intervention. We report a case of simultaneous bilateral spontaneous pneumothorax. As the patient had minimal symptoms and normal vital signs, he was conservatively managed. Follow‐up imaging showed resolution of the pneumothorax with no evidence of underlying lung abnormality or abnormal pleural communications. This rare case highlights the feasibility and safety of adopting a conservative approach in the management of bilateral spontaneous pneumothorax.
