Abstract

筑豊地域のC型肝炎患者481例を後ろ向き解析し,注射薬物使用歴(PWID)137例(28.5%)の臨床的特徴を非PWIDと比較した.PWIDは若年・男性・ジェノタイプ2型が多く,通院中断が高率で,そのためにSVR未確認が多かった.その結果,Intention-to-Treat解析ではPWID群のSVR率は低かったが(70.8% vs 91.9%),Per Protocol Set解析ではSVR率に群間差を認めなかった(95.1% vs 98.4%).また,精神科病院からの紹介が多く,治療開始時の肝線維化は軽度で,再感染は少数ながら認めた.人口当たりの市町村別分布は偏在が示唆された.医療・福祉・矯正・精神科の連携と継続支援が,地域のHCV排除に有用と考えられる.
