Abstract Posterior reversible encephalopathy syndrome (PRES) is a reversible neurological condition characterized by vasogenic edema, most commonly affecting the posterior regions of the brain. While its neurological and radiological features are well-documented, the psychiatric manifestations of PRES remain underrecognized. We present the case of a young postpartum woman who developed seizures, radiological findings consistent with PRES, and acute-onset psychotic symptoms. This case highlights the need for awareness of possible psychiatric features that may present with PRES, especially in vulnerable population such as postpartum patients.
