This case report provides the first evidence of reverse takotsubo cardiomyopathy precipitated by a tyramine reaction in the setting of monoamine oxidase inhibitor use.
Background Reverse takotsubo cardiomyopathy is a variant of takotsubo cardiomyopathy (TC) where the basal and midventricular segments of the left ventricle are akinetic. TC is typically precipitated by emotional stress; however, the exact pathogenesis of the disease remains unclear. Case Summary A 25‐year‐old woman, without any risk factors, presented with a hypertensive crisis and reverse takotsubo cardiomyopathy after a tyramine reaction. The symptoms and cardiomyopathy resolved completely. Discussion This report is the first to describe a case of takotsubo cardiomyopathy precipitated by a tyramine reaction in the setting of a monoamine oxidase inhibitor. This paper will include a pharmacological and pathophysiological review of the described case.
