Key Result

18F-FDG and 18F-FAPI-74 PET/CT imaging in a 47-year-old man with primary extraskeletal Ewing sarcoma demonstrated intense FDG avidity but relatively low FAPI uptake.

Key Points

  • The study aims to compare the imaging characteristics of 18F-FDG and 18F-FAPI-74 PET/CT in extraskeletal Ewing sarcoma.
  • Conducted PET/CT imaging using both 18F-FDG and 18F-FAPI-74 radiotracers.
  • Monitored tracer uptake in a primary extraskeletal Ewing sarcoma located in the posterior mediastinum.
  • Confirmed diagnosis through biopsy pathology.
  • 18F-FDG imaging showed intense tracer uptake in the tumor mass.
  • 18F-FAPI-74 uptake was low and limited to the tumor's peripheral rim.
  • Biopsy results validated the presence of Ewing sarcoma.

18F-FDG and 18F-FAPI-74 PET/CT imaging demonstrated discordant tracer uptake in a rare case of primary extraskeletal Ewing sarcoma.

Abstract

Primary extraskeletal Ewing sarcoma is rare. This report presents a 47‑year‑old man with primary extraskeletal Ewing sarcoma located in the posterior mediastinum, who underwent both 18 F-FDG and 18 F-FAPI-74 PET/CT imaging. The scans demonstrated a notable discordance in tracer uptake: the mass exhibited intense FDG avidity, while FAPI uptake was relatively low and confined to the peripheral rim. The biopsy pathology confirmed the diagnosis of Ewing sarcoma.

Hui et al. (2026) conducted a case report in Primary extraskeletal Ewing sarcoma (n=1). 18F-FDG and 18F-FAPI-74 PET/CT was evaluated. 18F-FDG and 18F-FAPI-74 PET/CT imaging in a 47-year-old man with primary extraskeletal Ewing sarcoma demonstrated intense FDG avidity but relatively low FAPI uptake.
