Key Points

  • The research aims to assess the implications of discordant imaging findings in diagnosing parathyroid carcinoma and adenoma.
  • Utilized 99mTc-sestamibi scintigraphy for imaging evaluation.
  • Employed ultrasound and computed tomography for comprehensive assessment.
  • Applied bilateral internal jugular venous parathyroid hormone sampling to inform surgical decisions.
  • Identified cases where parathyroid carcinoma appeared negative on scintigraphy.
  • Established that additional imaging techniques improve diagnostic accuracy.
  • Recommended preoperative evaluation for thyroid nodules to prevent secondary surgeries.

Abstract

Parathyroid carcinoma can appear negative on 99mTc-sestamibi scintigraphy; therefore, comprehensive evaluation with both ultrasound and computed tomography is essential. In cases with discordant imaging and multiple lesions, bilateral internal jugular venous parathyroid hormone sampling serves as a decisive adjunct to guide en bloc resection when carcinoma is suspected. Preoperative evaluation for concurrent thyroid nodules is recommended during parathyroid surgery planning to avoid the need for secondary neck procedures.
