Multilevel congenital anomalies of the iliac system are strongly associated with ischaemia, whereas isolated agenesis may be haemodynamically compensated and clinically silent. Recognition of these variants is important for accurate diagnosis and safe surgical planning, particularly in vascular, orthopaedic and transplant procedures.
