Abstract

【要旨】86歳女性．特に誘引なく左臀部から大腿部痛が出現し，前医で左恥骨骨折，仙骨骨折と診断された．保存加療が行われていたが，経時的に症状が増悪し日常生活困難となったため当院紹介となった．脆弱性骨盤骨折（以下FFP）FFP分類Type 3bと判断し受傷後約3ヶ月で逆行性恥骨上枝スクリューおよび腰椎骨盤後方固定術による骨接合術を施行した．術後経過は良好で歩行器歩行レベルでリハビリ転院したが，術後6週目に立位時に大腿内側に疼痛が出現し，歩行困難となった．単純CTで精査したところ恥骨スクリュー先端が骨外に逸脱しており，それによる閉鎖神経障害が疑われた．全身麻酔下に恥骨スクリュー抜釘術を施行し，その後疼痛誘発は改善し歩行可能となった．【結後】逆行性恥骨上枝スクリューの骨外逸脱は術中透視では十分に評価困難なこともある．閉鎖神経障害は注意する合併症の一つである．
