Abstract Background and aims The number of patients with acute ischemic stroke is increasing, yet effective measures for long-term standardized management of these patients are still lacking. The follow-up model led by advanced practice nurses provides a new approach for the management of such patients. Methods A follow-up programme led by advanced practice nurses was developed, encompassing both in-hospital and post-discharge management. In-hospital management involved advanced practice nurses providing structured health education and guidance during the patient’s hospital stay. On the day of discharge, an individualised home self-management plan was formulated for each patient. Post-discharge management included follow-ups conducted by advanced practice nurses at 1 month, 3 months, 6 months, and 1 year after discharge via telephone, WeChat, and online platforms. Patients were reassessed during these follow-ups; those with issues received appropriate education and guidance, with timely communication to team members and assistance in scheduling outpatient appointments when necessary. Results A total of 198 patients were included. After one year of intervention, the registration completion rate was 100% (198/198), the follow-up completion rate was 100% (198/198), the mortality rate was 2.0% (4/198), and the recurrence rate was 6.6% (13/198). Medication adherence rates were 94.3% (183/194) for antithrombotic drugs and 92.8% (180/194) for statins. Conclusions A follow-up model led by advanced practice nurses shows promising application prospects in patients with acute ischaemic stroke. This model can provide patients with comprehensive medical care support, reduce the recurrence rate, and improve medication adherence, making it worthy of promotion and application in clinical practice.
