Abstract

본 연구는 전시 및 평시 작전 환경에서의 군 의료 후송(MEDEVAC) 우선순위 결정과 중환자실(ICU) 병상 배분 문제를 대상으로, 동형암호(HE)를 활용한 개인정보 보호 기반 의사결정 구조를 제안한다. 기존 MEDEVAC 모형은 부상 병사의 임상 정보를 평문으로 공유하는 것을 전제로 하여 군 보안과 의료정보 보호 요구를 동시에 충족하기 어렵다는 한계를 가진다. 이에 HE 연산 제약을 반영한 polynomial logit 구조를 설계하고, 암호화된 위험도 점수의 순위 안정성을 이론적으로 분석한다. 특히 Top-k 경계 간격과 암호 노이즈 간의 관계를 정리하여, 암호 환경에서 우선순위 집합이 유지될 수 있는 충분조건을 제시한다. 모의실험 결과, 설정된 노이즈 범위 내에서 HE 기반 위험도는 평문 기반과 높은 순위 상관을 보였으며, MEDEVAC 및 ICU 배분에서도 유사한 결정이 도출되었다. 다만 연산 깊이 초과나 노이즈 누적 시 순위 왜곡 가능성이 존재한다. 본 연구는 상급 제대가 개별 임상 정보에 접근하지 않고도 암호화된 위험도 값만으로 우선순위 결정을 수행할 수 있음을 보이며, 군 의료 지휘통제 환경에서 정보 보호와 작전 효율성을 동시에 고려한 암호화 기반 의사결정 구조의 가능성을 제시한다.
