Posterior capsular opacification (PCO), the most common late postoperative complication of cataract surgery, is managed with Nd:YAG laser posterior capsulotomy. While intraocular lens (IOL) pitting is a frequently observed complication following this procedure, we present a unique case of extensive IOL pitting accompanied by cracks resembling crystals and fundus examination showing old branch retinal vein occlusion (BRVO). This case highlights the importance of precise laser beam focusing on the posterior capsule, ensuring posterior focus, and using appropriate laser energy settings during capsulotomy to minimize damage to the IOL. Also emphasizes complete ocular examination along with posterior segment evaluation.
