Abstract

2008～2022年に当院で原発性非小細胞肺癌に対し区域切除を施行した375例のうち，残存肺葉内の局所再発，二次原発性肺癌に対し局所治療を施行した8例を後方視的に検討した．区域切除から局所治療までの期間中央値は57ヵ月で，治療は残存肺葉切除4例，区域切除1例，sbrt3例であった．手術は周術期死亡や重篤な合併症はなく安全に施行され，病理診断は局所再発3例，二次原発性肺癌2例であった．sbrtは併存疾患やps不良2例，他肺葉切除既往1例で選択された．局所治療後の観察期間中央値は29ヵ月で，死亡は2例（肺癌死1例，他癌死1例）であった．区域切除後の残存肺葉内に生じた局所再発または二次原発性肺癌に対する局所治療の臨床成績は妥当であった．手術は安全に施行可能であり，sbrtも有効な治療選択肢となり得る．
