Clear aligner therapy has become a popular treatment modality among general dental practitioners, offering numerous benefits and often forming part of a broader restorative plan. However, this rise in use has been accompanied by a notable increase in patient-reported concerns. Complications can arise at various stages of treatment, underscoring the importance of clinicians maintaining professional vigilance. An over-reliance on remote diagnostics and digital tools must be avoided and core responsibilities such as obtaining informed consent, keeping comprehensive and accurate records, and providing transparent communication should be upheld. Patients have frequently raised concerns stemming from inadequate communication, especially regarding compliance with aligner wear, the use of intra-oral elastics, the need for composite attachments and interproximal reduction. The significance of attentive patient monitoring and clear guidance about retention protocols cannot be overstated. It is also essential that treatment remains within the clinician's scope of practice and is delivered by practitioners who are adequately trained, competent and appropriately identified. Where problems are anticipated, the involvement of a specialist orthodontist may help bridge gaps in knowledge and support high standards of patient care.
