Results of Sutureless Scleral Fixation (Yamane Technique) Performed in a Secondary Healthcare Center: Retrospective Study

Abstract

Objective: To evaluate refractive results and complications in patients who underwent scleral fixation with the Yamane technique due to aphakia or intraocular lens (IOL) subluxation at a secondary healthcare center. Material and Methods: This retrospective study was conducted with 27 patients who underwent surgery with the Yamane technique due to aphakia or IOL subluxation at a secondary healthcare center. The patients' best-corrected visual acuity (BCVA), refraction values, and intraocular pressure measurements were evaluated preoperatively and at the postoperative first and sixth months. The intraoperative, early, and relatively late complications of the cases were examined. Results: Twenty-three (85.1%) of the patients underwent surgery due to aphakia, and four (14.9%) due to IOL subluxation. The mean BCVA values were 0.72 ± 0.11 logMAR preoperatively and 0.22 ± 0.09 logMAR at the postoperative sixth month (p0.05). Concerning complications, intraoperative fracture of the IOL haptic during the placement of the trailing haptic into the 26-gauge needle was observed in two (7%) cases, while early postoperative intraocular pressure increase, postoperative IOL dislocation, cystoid macular edema, and pseudophakic bullous keratopathy were detected in one case (3.5%) each. Conclusion: This study showed that the Yamane technique can be applied in secondary healthcare institutions for cases of aphakic or IOL subluxation, and that it is an effective and reliable surgical procedure.
