Abstract

Key Points Platelet dysfunction in cirrhosis is influenced by multiple factors, including the severity of liver disease, the acute versus chronic setting, the presence of bacterial infection and organ failure. Thromboelastography (TEG) with Platelet Mapping has identified impaired platelet responsiveness despite elevated levels of von Willebrand factor (VWF) and factor VIII (FVIII), challenging the concept of a universally preserved or compensated haemostatic balance in cirrhosis. Although global haemostatic assays are promising for prognostic assessment, they still require validation against hard clinical outcomes before they can be reliably used to guide bleeding risk stratification in clinical practice.
