Patients' safety has received widespread interest, particularly after the landmark article by the United States Institute of Medicine titled 'To Err is Human' in 2000.1 Before reaching the public eye, the field of anaesthesia was at the forefront of patients' safety with modern advances in the field beginning in the 1970s.2 The result of these innovations included improved outcomes, development of standards and the establishment of patients' safety bodies including the Anesthesia Patients' Safety Foundation in 1985.
