Abstract

要旨 【背景】 極度の低栄養状態では,低血糖やたこつぼ型心筋症などを発症することが報告されているが,栄養管理に関しては十分に確立されていない。本報告では,body mass index(BMI)10kg/m 2 以下の意識障害を伴った極度の低栄養状態の低血糖,たこつぼ型心筋症合併患者に対して間接熱量計を用いて栄養管理を行った1例を報告する。 【症例】 43歳女性,BMI 8.6kg/m 2 。意識障害で救急搬送された。来院時血糖値は6mg/dL,トリグリセリド4mg/dL,循環不全,肝逸脱酵素の上昇を認めた。遷延する低血糖に対して糖液の持続投与を行った。第4病日にたこつぼ型心筋症と診断され,脂質を含む静脈栄養と経腸栄養が投与された。間接熱量計を測定すると,第4病日のエネルギー消費量は552kcal/日であった。エネルギー消費量よりも多いエネルギー投与量を目標に設定し,電解質異常の補正を行うことで,段階的に投与エネルギー量を上昇することが可能となり,1週間後には全身状態は改善した。 【結語】 極度の低栄養状態に対して,低血糖を回避するために糖液の持続投与を行い,低トリグリセライド血症およびたこつぼ型心筋症の改善のために脂肪乳剤の持続投与を行った。間接熱量計を用いた目標投与エネルギー量を設定することで,有害な合併症を来すことなく改善した1例を経験した。
