Abstract

Introduction and Objective: Approximately 40% of youth with type 1 diabetes (T1D) have obesity. T1D management presents barriers to engagement in standard intensive health and behavioral lifestyle treatments (IHBLT) for obesity. The current study is a secondary analysis of a feasibility and acceptability study testing an adapted IHBLT for adolescents with T1D. Methods: Adolescents with T1D and with, or at risk for, obesity (BMI70th %ile) completed the Type 1 Diabetes Adolescents for healthier Lifestyles (T1DAL) Study, a 16-week, adapted IHBLT for adolescents with T1D. All participants completed the following measures at Weeks 0 and 16: height, weight, continuous glucose monitor (CGM) wear over 10 days, and psychosocial measures (e.g., depression, diabetes distress, fear of hypoglycemia, disordered eating). Data were analyzed using descriptive statistics and independent samples t-tests in SPSS. Results: Seventeen adolescents (age=15.1yrs; 52.9% female; 76.5% white) completed the study. Weight remained stable. There was a decrease in depression symptoms (t(32)=1.45, p=0.018, ŋ2=0.06). There was also a decrease in percent time with low blood glucose (i.e., 70mg/dL; t(26)=0.914, p=0.049; ŋ2=0.03) and an increase in CGM usage (t(26)=-1.948, p=0.024, ŋ2=0.13). While not statistically significant, there was a medium to large effect size with regard to decreased percent time with high blood glucose (i.e., 180mg/dL; ŋ2=0.05), decreased diabetes distress (ŋ2=0.04), and decreased fear of hypoglycemia (ŋ2=0.04). Though not statistically significant, 60% of participants improved or did not change their time in target blood glucose range. Conclusion: Results provide a signal of improvement in weight stability, glucose stability, and improved psychosocial outcomes among participants of T1DAL. A larger sample and randomized controlled trial are needed to test the efficacy of T1DAL. Future research is needed to determine dose and mechanisms of change specific to IHBLTs for youth with T1D.
