This case demonstrates the utility of intravenous lipid rescue for local anesthetic-induced cardiovascular toxicity during prolonged resuscitative efforts.
In Brief We describe a case of successful resuscitation with an IV lipid infusion of local anesthetic-induced cardiovascular toxicity after supraclavicular brachial plexus block with mepivacaine and bupivacaine. Lipid therapy was initiated after 10 min of unsuccessful resuscitation and resulted in restoration of cardiovascular activity and hemodynamic stability. This case illustrates the utility of IV lipid therapy in the treatment of local anesthetic toxicity. IMPLICATIONS: We report a case of successful resuscitation with intralipid infusion after cardiovascular collapse after supraclavicular block with ropivacaine and mepivacaine. This case demonstrates the utility of lipid rescue for presumed local anesthetic toxicity during prolonged resuscitative efforts.
