A BSTRACT Objective: This study compares the efficacy of erbium: yttrium-aluminum-garnet (Er:YAG) laser versus conventional surgical bur in terms of postoperative pain, edema, trismus, and patient satisfaction. Materials and Methods: Twenty participants (18–30 years) with bilateral, asymptomatic impacted mandibular third molars were divided into two groups: Group A (conventional bur) and Group B (Er:YAG laser). Pain, edema, and trismus were assessed at multiple time points. Patient satisfaction was measured via a questionnaire. Results: Post-surgery, Group B experienced lower pain scores, reduced edema, less severe trismus, and higher patient satisfaction scores. Conclusion: Er:YAG laser-assisted third molar extraction offers significant advantages over conventional bur methods, particularly in postoperative pain, edema, and trismus, along with higher patient satisfaction.
