Abstract Vatarakta, a classical disease described in Ayurvedic texts, presents with clinical features that are remarkably similar to gouty arthritis. It is characterized by the morbid amalgamation of Vata dosha and Rakta dhatu, manifesting as excruciating pain (shoola), inflammation (shotha), and burning sensation (daha) in the joints. This case report details the successful management of a 52-year-old female patient diagnosed with acute gouty arthritis, presenting with severe pain in the right ankle joint. The Ayurvedic diagnosis was established as Vata-Pitta dominant Vatarakta. A unique and specific treatment protocol was designed, including internal medications like Amritadi Guggulu and Kokilakshakam Kashayam, along with a topical herbal paste (Lepa). The patient was also counselled on a strict dietary regimen (Pathya). Over an 8-week period, the patient experienced complete remission of symptoms, and her serum uric acid levels reduced from 8.9 mg/dL to 6.2 mg/dL. This case illustrates the efficacy of a targeted, multi-faceted Ayurvedic protocol in managing acute gout and its underlying metabolic cause.
