Cardiovascular disease (CVD) affects more than 125 million Americans, accounting for an estimated 48% of the adult population living with some form of CVD, including coronary heart disease, heart failure, stroke, or hypertension.1 Within the last decade, the American Heart Association increased its emphasis on health equity, as existing cardiovascular health disparities were exacerbated by the pandemic, negatively impacting outcomes.2 Noncommunicable or chronic diseases, including CVD, continue to be the leading causes of morbidity and mortality worldwide, disproportionately impacting underserved populations.1,3 Health disparities remain pervasive, with racial, ethnic, and socioeconomic groups experiencing a higher incidence and more severe consequences of cardiovascular conditions due to factors such as limited healthcare access, socioeconomic barriers, and systemic inequities.1,3,4 Cardiovascular health inequities are not only unique to low- and middle-income countries but also an issue among individuals living in high-income countries.4,5 Nurse-led innovations such as community-based clinical practice and the use of health technology are at the forefront of cardiovascular care management.6,7 The Role of Technology in Cardiovascular Care Management Nurses are well positioned to manage cardiovascular conditions across diverse healthcare settings. With a focus on patient-centered care, nurses are often the first point of contact for patients and can provide continuous monitoring, education, and advocacy.8 Nursing interventions are well suited to address the multifaceted nature of cardiovascular care, which requires not only medical management but also psychosocial support, lifestyle modification, and community-based resources.6,8,9 The future of nursing innovation in cardiovascular care lies in the strategic integration of health technologies, such as wearable electronic devices, and the use of artificial intelligence (AI) for clinical practice.10–12 These tools enhance communication between patients and care teams, enable real-time monitoring, and facilitate data-informed decision making.11 For example, wearables that track blood glucose levels, blood pressure, electrocardiograms, and other vital assessments allow nurses to identify changes early and adjust treatment plans promptly, reducing complications.12 Artificial intelligence is also transforming clinical workflows by supporting remote monitoring and delivering evidence-based recommendations through decision support systems. Nurses are well positioned to lead the design, implementation, and evaluation of these technologies, ensuring they are patient centered and equitable. By leveraging AI, nurses can deepen clinical expertise, enhance care efficiency, and drive better health outcomes, especially in underserved settings.11,13 Innovations in Chronic Care Through Community-Based Clinical Practice Innovative clinical models rooted in community engagement are transforming chronic disease management by bringing care directly to where patients live, work, and gather. Mobile health clinics, home health services, and partnerships with trusted community institutions such as churches, schools, and barbershops have emerged as effective strategies for improving access to care and clinical outcomes, particularly in underserved populations.8,14 Clinicians working in community settings are often on the frontlines of care, providing screening, disease education, medication management, and follow-up for conditions such as hypertension, diabetes, and heart failure.7 Nurses play a pivotal role in delivering care within these community frameworks, combining clinical assessment with education and care coordination. Meeting patients in their communities allows clinicians to build trust, identify barriers to adherence, and tailor interventions in real time.14 This approach has been particularly effective in addressing long-standing mistrust in healthcare systems among historically marginalized populations.6,7,15 For example, community-based cardiovascular programs such as those that partner with barbershops and beauty salons have been successful in increasing participation and improving management of chronic conditions such as hypertension.14,15 Nurse-led self-management education is a core component of these models. Teaching patients to monitor symptoms, manage medications, and make informed lifestyle decisions empowers them to take an active role in their health.7 Clinically, this has led to improved patient understanding, improved self-management, and fewer hospitalizations for patients with chronic cardiovascular conditions.12 Ultimately, integrating community-based strategies into clinical care delivery represents a shift toward more equitable, accessible, and patient-centered chronic disease management.15 Implications for Cardiovascular Nurses Nursing innovations are essential for reducing cardiovascular health disparities and improving outcomes. Integrating health technologies such as wearable devices and AI with community-based care models enhances prevention, monitoring, and self-management, particularly in underserved populations.7,9,12,13 Nurses must lead efforts to expand these innovations, especially in rural and resource-limited settings. Enhancing nursing education to include digital health and AI training is critical for preparing the workforce to meet evolving healthcare demands.13,16 In addition, policy advocacy remains key. Nurses can influence equitable access to care and technology, ensuring resources reach the communities most affected by CVD.8 Continued investment in nursing-led research and interdisciplinary collaboration is vital to designing culturally responsive, evidence-based solutions. As the healthcare landscape evolves, nurses must remain central to advancing health equity through innovation, leadership, and community engagement.4 Looking ahead, sustained investment in nursing-led research on CVD prevention is vital to uncover evidence-based solutions that address the root causes of cardiovascular disparities. Interdisciplinary collaborations with community leaders, technologists, and policy makers will be essential to designing culturally responsive interventions that reflect the lived experiences of diverse populations. As the healthcare landscape continues to evolve, nurses must remain at the forefront of advancing health equity through innovation, advocacy, and leadership in cardiovascular care.8
