Abstract Objectives We report a case of a 40-year-old female patient admitted to our department with abdominal pain in the right lower quadrant associated with abdominal tenderness, leukocytosis, and elevated inflammatory markers. She was observed in the hospital with similar symptoms in the past with successful medical treatment. The current literature is also reviewed. Methods The abdominal ultrasound failed to visualize the appendix. However, the CT scans revealed that the appendix was located deep in the pelvis with an atypical, irregular, thickened wall with pericaecal fluid. Laparoscopic appendectomy was completed. The specimen showed features of diverticulitis of the appendix, as also proved histopathologically. We describe the successful surgical treatment of a rare pathology imitating acute appendicitis, providing reassurance about the effective management of appendiceal diverticulitis. Results The postoperative course was uneventful, and the patient was discharged on the second postoperative day in optimal clinical conditions. Conclusion Diverticulitis of the appendix is an uncommon cause of pain in the right lower quadrant. Understanding its clinical behavior is essential for managing it, whether it presents symptomatically or is an incidental finding.
