Abstract 60/male Presented to OPD with swelling near previous laparotomy scar mark. Reducible swelling. Increases in cough. Past history of paramedian incision laparotomy for perforated appendicitis. On examination – 7 × 6 cm defect near the scar mark. CT shows a paramedian ventral hernia. Plan – eTEP RS with unilateral TAR. Surgery procedure 3 port eTEP RS. Retrorectus space creation. Cross over to the opposite side. Dissection of peritoneum over scar mark. Opening of incisional hernia sac. Adhesiolysis of sac from contents. Dissection of defect on the superior and inferior aspect. Peritoneal flap creation. Right side TAR. Closure of peritoneum without tension. Closure of defect with nonabsorbable barned suture. Placement of 20 × 15 cm mesh. Strapping after surgery Operative time 180 minutes. Blood loss – 100cc. Patient was discharged on the 5th post-op day.
