Chavali , Carter MikesellCarter Mikesell , and Jihad KaoukJihad Kaouk View All Author Informationhttps://doi.org/10.1097/01.JU.0001009496.54470.10.01AboutPDF ToolsAdd to favoritesDownload CitationsTrack CitationsPermissionsReprints ShareFacebookLinked InTwitterEmail Abstract INTRODUCTION AND OBJECTIVE: With the increasing utility of partial nephrectomy (PN) for the management of suspicious renal mass, this study sought to evaluate the utility of three commonly used scoring systems, including RENAL, PADUA, and the more recently introduced Simplified PADUA RENAL (SPARE), in predicting the perioperative and longer-term functional outcomes following robotic PN (RPN). METHODS: A retrospective review was performed on the prospectively maintained, IRB-approved database to identify 2003 patients that underwent RPN for the management of suspicious renal mass between January 2006 to July 2023. Preoperative radiological images, such as Computed Tomography (CT) and/or Magnetic Resonance Imaging (MRI) were successfully retrieved for 1816 patients and were reviewed by at least two physicians. The tumors were categorized into different complexities based on the different scoring systems. Statistical analysis was performed using R Project for Statistical Computing. RESULTS: The RENAL scoring system classified 829, 771, and 216 patients into the low, intermediate, and high complexity categories. The PADUA and SPARE, on the other hand, categorized 785 and 1176 cases as low, 714 and 532 intermediate, and 317 and 108 high-complexity tumors, respectively. Regardless of the scoring system, the three complexity groups were similar in their baseline clinicodemographic characteristics, including the preoperative renal function. Low-complexity tumors were associated with faster operating time, shorter ischemia time, less intraoperative blood loss, and lower risk of postoperative complication. Despite the differences, our AUC analysis identified the relatively poor predictive capacity of the scoring systems towards various perioperative outcomes, including for the trifecta and pentafecta of PN. We did not identify any statistically significant differences in the rate of renal function preservation at 12 months following surgery across the different complexity groups. CONCLUSIONS: Despite the utility of the scoring systems in communicating potential surgical complexity, we have yet to find sufficient evidence towards their roles in predicting perioperative outcomes and long-term functional outcomes in our cohort of RPN.
