Abstract Background Crohn’s disease with perianal involvement is a more aggressive phenotype, accounting for approximately one-third of patients, and is associated with high rates of hospitalization and surgery. Diagnosis involves clinical, laboratory, endoscopic, histological, and radiological data. In this context, a proctological examination under sedation facilitates identification of the affected anatomy, but does not assess its functionality. The proposed study was an anorectal manometry study for sensory-motor correlation of the anal canal, given the pelvic floor damage. Methods This cross-sectional, descriptive, and quantitative study was approved by the Research Ethics Committee of the Faculty of Medicine of Botucatu - Brazil. The sample consists of 20 patients who signed the informed consent form and underwent conventional manometry during the follow-up period at the Inflammatory Bowel Disease Outpatient Clinic at Hospital of the Clinics – Botucatu Medical School, from August 2022 to January 2025. Results The profile studied was slightly higher in the male population, at 55%, with a mean age of 41.5 years, 70% white, and a mean time since diagnosis of 9 years. 35% presented some type of comorbidity such as hypertension or diabetes, and of these, 65% presented clinical and endoscopic remission of the disease. The results show that 90% of the operated sample had some manometric alteration, and in 81.82%, manometric alterations occurred even after healing of the fistula diagnosed by pelvic MRI. Conclusion Despite the sample size, there is a tendency toward earlier diagnosis of anorectal functional alterations, even in asymptomatic patients, reinforcing the hypothesis of this tool’s contribution to the assessment of anorectal disease. Further studies, including high-resolution manometry, should be conducted to confirm this trend, which could support surgical planning decisions and the introduction of new therapies. References: 1.
