Background: Crohn’s disease (CD)-related surgery may impact pregnancy outcomes. Methods: We utilized the prospective PIANO cohort to investigate pregnancy outcomes among women with CD by prior surgery via bivariate and multivariate analyses. Results: Among 1046 women with CD, 409 (39%) had prior surgery. Those with prior surgery had increased disease activity ( p < 0.001). On multivariate analysis, only disease activity (OR 1.14, 95% CI 1.06-1.22) and ileal disease (OR 2.19, 95% CI 1.26-3.83) were associated with preterm birth; not prior surgery. Conclusions: Prior surgery was not associated with adverse pregnancy or neonatal outcomes in CD.
