Background: Undifferentiated carcinoma with osteoclast-like giant cells (UCOGC) of the pancreas is a rare histological variant of pancreatic ductal adenocarcinoma. These tumours often present radiologically as solid or mixed solid–cystic masses, and their varied morphology can mimic benign cystic processes, leading to significant diagnostic challenges.
