District hospitals in Ghana play a crucial role in healthcare delivery, yet their operational efficiency is often questioned. This study employs comprehensive searches across multiple databases focusing on relevant studies from onwards. The methodology includes rigorous screening, data extraction, and quality assessment of included studies. Panel-data analysis reveals a significant variation in the effectiveness of district hospitals' systems, with some showing a moderate improvement over five years (p < 0. 05). The findings suggest that improved panel-data estimation methods can enhance the reliability and consistency of system performance measurements. District hospital managers should consider implementing these enhanced methods to better assess and improve their operational efficiency. district hospitals, Ghana, system reliability, panel data, healthcare systems Treatment effect was estimated with logit (pᵢ) =₀+^ Xᵢ, and uncertainty reported using confidence-interval based inference.
