Xanthogranulomatous inflammation is an uncommon form of chronic inflammation characterized by the accumulation of lipid-laden macrophages. While commonly observed in organs like the gallbladder and kidneys, xanthogranulomatous involvement of the peritoneum is exceedingly rare and poorly documented in the literature. We present an atypical presentation of rare xanthogranulomatous peritonitis as an abdominopelvic cystic mass mimicking ovarian malignancy. This case report aims to raise awareness regarding the inclusion of xanthogranulomatous peritonitis as a differential diagnosis in cases of abdominopelvic mass lesions where laboratory and imaging findings are not coherent.
