Medication errors (MEs) pose a significant threat to patient safety and healthcare quality worldwide. This narrative review explores the various types of MEs, including prescribing, transcribing, dispensing, and administration errors. It highlights key contributing factors such as inadequate communication, lack of standardized procedures, and insufficient knowledge among healthcare providers. The review also discusses the impact of these errors on patient outcomes and healthcare systems. Strategies for prevention are emphasized, including implementing technological solutions like computerized physician order entry, fostering a culture of safety, and providing continuous education and training. The importance of reporting systems and interdisciplinary collaboration in identifying and mitigating MEs is also underscored. Overall, this review aims to raise awareness about the multifaceted nature of MEs and advocate for comprehensive interventions to reduce their occurrence, ultimately improving patient care and safety.
